School & Sports Physical Exams

A school and sports physical checks that a child or teen can study and compete safely. At La Michoacana we do it walk-in, in Spanish, with the school form filled out and signed before you leave.

What gets checked?

Weight, height, blood pressure, vision and hearing, heart and lungs, spine and knee and ankle mobility, plus the questions about fainting, chest pain during exercise and family history of heart problems. That part of the questionnaire is what catches the most cases.

Which forms do you complete?

Your school or district form and the Texas school sports form, which at most high schools is the UIL form. Bring the paper they gave you; if you forgot it, tell us which school it is for.

What should I bring?

Parent or guardian ID, the student's vaccination record, glasses if they wear them and the names of any medications. If your child uses an asthma inhaler, bring it: it goes on the form and prevents problems at practice.

When should we come?

Before the season starts, not on the day of the first practice. July and August are when every family shows up; the rest of the year it takes a single visit, and we have evening and weekend hours, which is when most families can come without missing school or work.

Is it valid for daycare, adult sports or camps?

Yes. The exam is the same; only the form changes. Bring the one you were given, whether it is from the school, the league or the camp, and we complete and sign it during the visit.

Why come to La Michoacana?

We are on Spencer Hwy, at 6500 Ste 180, just off Highway 225 and minutes from Deer Park, South Houston and La Porte. We open seven days a week: Monday to Saturday from 9 AM to 9 PM and Sunday from 9 AM to 7 PM, so you can come after work or on the weekend without taking time off. No appointment and no insurance needed. Our whole team speaks Spanish, parking is free and the entrance is wheelchair accessible.

How payment works

You pay per visit, with no insurance involved. We take cash, debit cards, credit cards (Visa, Mastercard, American Express and Discover) and phone payments. We do not take checks. Ask about the price of your service when you arrive or over the phone: we tell you before we start, so you know what you are paying.

Most live in Pasadena, in ZIP codes 77505, 77504, 77506 and 77503. We also see families from Deer Park, South Houston, La Porte, Pearland and the rest of the Houston area. If you work at the industrial plants along Highway 225 or the Ship Channel, we are on your way home.
