Truck driver getting a DOT physical exam for a CDL license in Pasadena

If you drive commercial, the DOT physical is the only thing standing between your license and a week off the road. Most drivers who come to us in Pasadena walk in with the same question: "am I going to pass?". This guide explains exactly what gets measured, with what numbers, and what you can do before the visit so nothing catches you by surprise.

What the exam checks

The exam follows FMCSA rules and covers:

  • Vision: at least 20/40 in each eye and both together, with glasses if you wear them, plus a 70-degree field of vision in each eye. You also need to distinguish traffic signal colors.
  • Hearing: you must hear a forced whisper at five feet, or pass an audiometer test.
  • Blood pressure. The number one reason a certificate comes back short. Under 140/90 the certificate can run 24 months; between 140/90 and 159/99 it is usually one year; higher than that, the term gets shorter and medical follow-up is required.
  • Urinalysis, which looks for signs of kidney or sugar problems. It is not a drug test: that is separate.
  • Medical history and medications, including sleep apnea, diabetes and any recent surgery.

What to bring

  1. Photo ID and your CDL, or your permit if you are getting one.
  2. Glasses or hearing aids, if you use them. Without them, corrected vision cannot be certified.
  3. A list of your medications, with doses.
  4. If you take blood pressure medication, recent readings help.
  5. If you use insulin, the MCSA-5870 form filled out by the provider who manages your diabetes.
  6. If you use CPAP, your compliance report for the last 90 days.

How to show up in better shape

Three things that are in your hands and change the outcome of the day:

  • No coffee or energy drinks. They raise your pressure right before it gets measured. Same with smoking in the hour before.
  • Sleep and eat normally. Arriving fasted after many hours behind the wheel usually gives worse readings.
  • If you know your pressure runs high, take your medication as usual and come early, not after a twelve-hour shift.

How long the certificate lasts

The maximum is 24 months. With controlled high blood pressure, diabetes or another condition under follow-up, the examiner may issue it for 12 months or less. That is not a punishment: it is the window to check you again.

After the exam, remember that in Texas the certificate is reported to the state DPS so your license stays active. Always keep your copy.
